1.     Each player must complete a new registration form each year.

2.     Please ensure the player only signs their name at the appropriate signature line on the front of the form and that the Manager only signs next to the Club Officer Signature line.

3.     Every line the form MUST be filled in. If the player is New and they had no previous club registration, print "None" in the Club line below Last Registration.

4.     The Club Officer must provide us with a current phone number and address so that they can be contacted easily.

5.     Generally eighteen (20) is the maximum number of players permitted on the roster at any one time, unless your league has a separate rule that stipulates otherwise. In all Over-30 leagues unlimited players are permitted.

6.     If a player is released, he/she is not permitted to play for two weeks (outdoor) or one week (indoor).
Releases will only be accepted if they are accompanied by the current player pass and a signed release form submitted by the team the player is being released from.
If the team has lost the pass a fine of $10 will be incurred.

7.     A new player requires one picture and a photocopy of their ID.
An old player requires one picture or their old pass.
The photograph should be as close to a "head and shoulders" shot as possible.

8.     There is a $1000 penalty fine for forgery and/or deliberately misleading the ISSA.

9.      A player can only play for one team at a time, unless there is a standing agreement between the leagues involved.

10.   Liability and Medical insurance is mandatory for all leagues, without it claims will be immediately rejected. Please make sure your club is always in compliance.
